Information on forms, confidentiality and response rates.
- The deadline for forms to be returned for the quarter (1st October to 31st December 2016) is the 27th January 2017.
- A new survey form was introduced last quarter to run in parallel with the current form to assist in the switch over to the new form format in the next few quarters, see below for details of both forms.
